The challenge is that transfering evident knowledge (clinical guidelines) to care processes (clinical pathways) is difficult enough, but to do it across the planet with low latencies is even more difficult.
The Spec Clinical Meta Model merges the structures of Mesh Network Design with Clinical Guidelines and Clinical Pathways into a single model. It allows for data encoding in any format such as Health Level 7 (HL7) in version 3 or FHIR.

Therefore the model offers elements to describe an intersectoral pathway.
